It is obvious from the display below that styles did not turn out as the book shows. I shall experiment a bit in 042AStyles_for_Form_TS.html only for a short while. Many readers indicated mamy errors in this book; so spending too much time on this matter is not worthwhile.

I could not find the cause for the discrepancy for the two outputs. I spent a long time checking the script, and experimenting. No success!

In addition,

the indentation of 'Gender' is wrong and bold highlights are not shown.
'Choose File' and 'register' are not rounded corner link in the book.
